Follow these 3 simple steps to apply for your free product trial of Kogenate FS with BIO-SET:
- Download the enrollment form that is appropriate for you and complete the patient information section, in preparation for discussion with your HCP.
- Share the completed enrollment form with your healthcare provider and discuss if Kogenate FS with BIO-SET is right for you. If you both agree that it is, your healthcare provider will complete the prescriber information section.

*Please note that patients previously enrolled in the Kogenate FS Free Trial program and patients currently receiving Kogenate FS therapy are not eligible to participate in the program. In accordance with government statutes, patients covered in whole or in part by federal or state healthcare programs such as Medicare or Medicaid, cannot participate.
